I, the undersigned, being either the participant named above, or the parent/legal guardian of the participant (if under 18), hereby acknowledge the following:
- Give permission for the participant to be photographed, filmed, or recorded during club or competition activities, including matches, training sessions, presentations, and events.
- Consent to the use of such images, video, and audio recordings for: live streaming of matches or events; promotional and marketing material (e.g. social media, websites, printed materials); and club or association communications (e.g. newsletters, team announcements).
- Understand that no compensation or royalties will be paid for the use of these materials and that the club/association retains all rights to use the content in the context described above.
- Acknowledge that personal details (e.g. full name) will not be published without additional consent, except where required (e.g. team sheets, scoreboards).
- Understand that if I do not wish to consent, I must notify the Association in writing prior to participation.
